New Patient Registration

registrationThe doctors accept new patients who live or work within our practice area.  As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a new patient health questionnaire and fill in a Patient Registration Form. Once these have both been completed you will be offered a consultation with the doctor and/or nurse. 

In order to speed up the registration process, you can  complete our New Patient Health Check Questionnaire and Registration Form in advance of coming to the practice by downloading them from here. Please click on the links below to download and print the questionnaire and registration form;

Once you have completed both of these please bring them along to our Alderton or Orford surgeries in order to register.

Medical treatment is available from the date of registration.  Please contact reception for further information.

Practice Area

The practice covers the area known as ‘The Peninsula’, which stretches from the River Deben in the south to the River Alde/Ore in the north.  In total we cover an area of about 80 square miles.  This includes the newly added villages of Eyke, Bromeswell and Rendlesham.

Closure of Sutton Memorial Hall Doctor’s Session will be from the 1st April 2013.
Orford Surgery will open at 2pm on a Tuesday from the 1 January 2013.
